Weight Management Products - Prescription Instructions

If you have been authorised to prescribe weight loss products, as part of the purchase process you will be asked to complete your patient form and upload the patient consultation details securely. You will be asked to navigate to the patient form during the order process. Once submitted, please ensure you navigate back to the website to complete your order and check out. If this part of the process has not been completed, please use the Link for the form and upload area.

Step 1 -Navigate to the ePharmacy tab.

Step 2 – Fill the Patient Consultation Form in.

Step 3 - Upload forms.

Step 4 – You will receive the below message once the forms have been uploaded successfully.

Step 5 – Return to medfx ePharmacy page.

Please return to the medfx prescribe page using the ePharmacy tab in the mega menu.
